New COVID-19 Vaccination Clinic for Ages 65 and Older

Dear River Forest Residents,
The Village of River Forest is proud to again partner with Jewel Osco to provide a free COVID-19 Vaccine Clinic for individuals ages 65 and older. This clinic will utilize the Moderna vaccine. Please help us spread the word to River Forest residents in this age group who still need the vaccine. This is an additional vaccine clinic as the Village was able to secure 200 additional doses through its partnership with Jewel Osco. Please note... anyone who participated in the clinic on February 26 should not register for this clinic.
- When: Tuesday, March 2 (first dose) and Tuesday, March 30 (second dose). Appointments will be offered between 9 a.m. and 1 p.m.
- Who: Individuals age 65 and older. Please register only if you know you are available on March 2 and March 30 to receive both vaccine doses. Limited vaccines are available and sign-up will be offered while supplies last. Individuals who sign up and are not 65 or older will be unable to receive the vaccine.
- Where: River Forest Village Hall - 400 Park Avenue.
- How: If you are 65 or older and are interested in this opportunity, please click here to register. River Forest residents who need help registering or are unable to make arrangements for their own transportation please contact the River Forest Senior Response Team at 708-613-1642.
Appointments are available on a first-come, first-served basis and are limited based on the number of doses available. Once the event is full the Village will post a waitlist and if a dose becomes available because of a cancellation or missed appointment, the Village will contact people on the waitlist.
All individuals in Illinois age 65 and older can receive the vaccine regardless of insurance coverage or immigration status, including individuals who are undocumented.

Instructions
Please report to the River Forest Village Hall no more than 15 minutes before your appointment time. Participants will be directed where to park.
Only persons with scheduled appointments will be admitted. After receiving the vaccine, participants will be required to stay in designated areas for a 15-20 minute observation period before being allowed to leave.
All participants must wear a mask and maintain social distancing whenever possible.
Participants should:
- Bring a photo identification with birthdate.
- Bring a completed consent form. Participants who bring a completed copy will help to aid in the flow of the event. Anyone who is unable to print and complete the form ahead of time will be provided with a copy upon arrival.
- Bring an insurance card (including Medicare or Medicaid). This event is free.
- Wear short sleeve shirts/clothing that will allow for easy administration of the vaccine
